Determining Insulin Doses

On August 13, 2011 8 year-old Moriah was diagnosed with type 1 diabetes. I was with her when the nurse and nutritionist explained how to determine her insulin doses. It is quite a bit of math for an 8 year-old to have to do at every meal. I decided to turn it into a worksheet.
